Case Manager III - Coalinga

Type:                      Regular, Full- Time (Monday – Friday 8:00am – 4:30pm)

Reports to:            Family Stabilization Program Manager

SUMMARY: The Case Manger III will assist and enroll identified clients who are appropriate for DV services through the Family Stabilization Program, in partnership with the Department of Social Services. This position will provide intensive case management services to clients/families of the Coalinga and surrounding area, who are experiencing an identified crisis DV situation, or have barriers pertaining to an overall history of DV.

The Case Manager III will monitor each client’s case in order to ensure that clients are maintaining active participation within the FSP program. The Case Manager III will provide direct case services, including but not limited to: advocacy, addressing identified barriers- which can include housing, providing assistance in attaining needed services, goal setting, linkage to needed counseling for them and their children, evaluation of needs, and assists the client in making necessary community linkages in order to support their self-sufficiency goals, much of which is identified and reflected in their “plan of care”. The CM III, will have frequent and ongoing contact with each family, ranging from daily to weekly, depending on the level of need and progress in FSP, via phone and/or in person, along with service providers to determine the effectiveness of services provision. This position must work and communicate effectively with our County of Fresno partner agency, law enforcement, and the community; and will work closely and cooperatively with other MMC services staff to ensure satisfaction with services provided.  In addition to direct Case Management services, the CM III will also facilitate the required classes to those enrolled in the FSP program. 


Duties and Responsibilities:

  1. Complete intake/crisis assessments in order to determine eligibility for Family Stabilization and possible safe housing services.

  2. Work with the designated FSP Job Specialist to develop initial plan of care.

  3. Manage a full case load of up to 20 FSP clients who are DV survivors in need of ongoing services coordination, resources and support ensuring that case management advocacy & support necessary at meetings for clients; court, TDMs, etc.

  4. Facilitate Safe Group, Parenting and other classes, as needed, to enrolled participants of the FSP program.

  5. Identify barriers to services and develop strategies with the client to overcome them.

  6. Assist the clients in meeting their goals in their FSP “plan of care”, and providing clients with linkage to community based resources as needed.

  7. Communicates with Supervisor on a regular basis in regards to client status, plan, and overall wellbeing. 

  8. May conduct home visits with Public Health Nurse in order to develop case plans that will focus DV survivor and their children toward meeting and achieving their safety, legal, clinical and ongoing housing needs.

  9. Provides safety planning, in order to address safety issues and concerns.

  10. Meets with clients on a regular basis (minimum of 1X week) to monitor goal achievement, identify/address barriers to services and to make ongoing referrals for needed services.

  11. Maintains weekly communication with Fresno County assigned Job Specialist- in order to provide most accurate information pertaining to client’s progress within the Family Stabilization Program.

  12. Maintain accurate client records, as well as processing and submitting monthly progress reports to DSS JS on each client. 

  13. Keep case files maintained, up to date with case plans and programmatic needs that meet the developed project implementation plans and as per the agency requirements.

  14. Support child/parent activities focused on building positive family relationships.

  15. Works cooperatively, coordinates and supports the MMC/DSS FSP project, Transitional Projects, Emergency Safe House, Clinical staff, and Legal Services, as well as other areas of the MMC organization, to insure appropriate client referrals and participation in services.

  16. Maintain confidentiality of client records and information.

  17. Work with supervisory and management staff to achieve programmatic expectations, and support and screen emergency and transitional DV clients for appropriateness of services;

  18. Program implementation and development in order to meet/maintain compliance with grants requirements.

  19. Participates in case reviews, staffing, and family assessments- minimum 2Xs monthly.

  20. Works with required projects guidelines and follows agency policies and procedures.

  21. Personally reports to Department of Social Services all suspected child abuse and or neglect required by law and document accordingly.

  22. Other duties as assigned and necessary to achieve agency, programmatic, and client goals.

  23. Regular attendance and promptness are considered part of the job.

  24. The primary location for this position will be in Coalinga but will require travel at minimum 2Xs monthly for staff meetings, supervision, trainings etc.

Knowledge, Skills and Abilities:

  • Willingness and ability to work independently, with each family who has been affected by DV.

  • Ability to make decisions which may have moderate to major impact on the operation of programand/or agency (general operations to funding).

  • Capacity to problem solve, handle crises, and work with families and children of various cultures from low-income backgrounds.

  • Approach to working with families in an empathic, nonjudgmental, respectful, and professional manner.

  • Ability to understand the needs of families and a willingness to address their concerns with action designed to create change.

  • Demonstrate ability to work with sensitivity and without discrimination towards people of diverse cultures, races/ ethnicities, socio-economic positions, ages, religions, genders, physical/mental challenges/ disabilities and sexual orientations.

Education and Experience: 

  • Experience and knowledge of Case Management services.

  • Bachelor's Degree.

  • Two years of case management experience in domestic violence or human services field.

Language Skills:

  • Bilingual abilities required:Spanish/English

  • Ability to effectively communicate, in Spanish/English, with staff, volunteers, interns, clients, and the public. Ability to read, analyzes, and interprets professional journals or governmental regulations.

  • Ability to write reports, business correspondence and procedural manuals.

  • Ability to effectively present information and respond to questions from groups of managers, clients, and the general public.

CERTIFICATES, LICENSES, REGISTRATIONS:

  • Possession of a valid California driver's license and proof of liability insurance on personal auto.Must be insurable at all times at standard rate by set by MMC insurance carrier.

  • Must pass tuberculosis test, drug test and fingerprinting test.
